Injuries in the Workpalce

Disclaimer: Please be advised that not all injuries that occur in the workplace are automatically covered by workers' compensation insurance. Coverage may depend on factors such as the nature of the injury, the circumstances surrounding the incident, and applicable state or federal laws. It is important to report any workplace injury promptly to Union Human Resources. 

When an employee has a work-related incident or injury, it MUST be reported to Union HR within 3 business days.

Minor injuries that do not need medical treatment can be reported to Union HR. This is necessary in the instance where treatment may be needed in the future. 

 

If the incident is an emergency, call 911 immediately or take action to seek the emergency treatment at the nearest emergency facility.

Within 24 hours of the injury, report the injury to the Union's Workers' Compensation Provider, SFM, by:

  • Calling the Nurse Triage Line at (855)-675-3501 (choose 1 from available options) and follow their care instructions;

Or

  • Report the injury online. Policy number: 128341.201

You will need the following documents when you seek treatment:

 

If the incident requires any non-emergency medical care (including urgent care) the employee should take the following steps: 

1. For injuries requiring medical attention but not an emergency room visit (e.g., sprains, cuts, minor burns) the employee may seek care at an urgent care center. 

2. Before or after visiting urgent care, the injury must be reported to the Union's Workers' Compensation Provider, SFM, by:

  • Calling the Nurse Triage Line at (855)-675-3501 (choose option 1), following the care instructions provided.
